Joining the Practice
We can only accept new patients who live in our practice area. You will be required to come into the surgery to register bringing your NHS number, utility bill and identification such as a passport. We will ask you to complete and sign your NHS medical card which will allow us to register you with the Practice and obtain your medical records from your previous Practice. Should your NHS Medical Card be unavailable, you will be requested to complete and sign an Application Form.
Your Medical Records
As there will be a delay in obtaining your medical records from your previous Practice, we will ask you to complete a Patient Questionnaire for yourself and each member of your family who are registering with the Practice. This will advise us of your relevant lifestyle and medical history which will be most helpful until your medical records arrive.

Invitation to attend for an Entry Medical
We will invite you to attend a short Entry Medical with our Practice Nurse or other Health Professional. We will ask you to bring a small specimen of urine with you for testing. Your blood pressure, height and weight will be recorded as well. We will give you a special container in which to provide the urine specimen. Information about any chronic diseases from which you suffer will be recorded as well.
Registration Status
You will be registered with the Practice rather than an individually named Doctor. You will be allocated a usual Doctor but you may see any of the Doctors at the Practice by appointment. We also have two salaried female Doctors and a GP Registrar with whom you may consult as well.
Continuity of Care
Wherever possible, it is preferred that you see the same Doctor for your medical care as that Doctor will become familiar with your medical history. There will be times when your usual Doctor is away from the Practice. In this instance, another Doctor (a “Buddy Doctor”), will see you when your own Doctor is not available.
